What should an intake evaluation measure?

Look at the complete journey, including the effort added for each role.

Choose a bounded pathway

Define eligible visits, access modes, the care-team reviewer, and escalation responsibilities. Agree on the question the evaluation should answer before selecting a metric.

Measure completion and access

Record eligible visits, started intakes, completed intakes, abandoned flows, and the help required to complete them. Include caregiver and staff-assisted paths rather than treating them as exceptions.

Measure effort across roles

Compare patient entry effort, nurse review time, repeated questioning, handoff completeness, and staff support. Count added work alongside avoided work; do not infer reduced waiting time or headcount from a shorter form.

Follow updates through review

Measure receipt, time to opening, completed review, reassessment, and patient contact as distinct events. A submitted update alone does not demonstrate that it has been acted on.

Agree on decision criteria

Set thresholds and a stopping rule with the clinical team. Review the findings, including failed or incomplete paths, before expanding the scope. This guide makes no quantified MediLink outcome claim.
